Aging in Place Program
Helping seniors remain safe, independent, and supported at home
Sometimes the best place to be is right where you are. Our Aging in Place Program helps seniors remain safely in their homes after health changes, hospitalizations, or functional decline.
Services May Include
• Licensed Occupational Therapy home safety assessment
• Professional care management and coordination of services
• IADL support including meal preparation, errands, and light housekeeping
• Optional decluttering and organizing to improve safety and accessibility
• Discharge planning beginning at Day 60 to ensure long-term support

Move Management Program
Guided transitions to the right care community
When living at home is no longer the safest option, our Move Management Program helps families navigate the complex process of transitioning to the right care environment.
We coordinate every step — from selecting the right community to managing the move itself.

Our structured 90-day model ensures that seniors and families receive the time, guidance, and professional oversight needed to achieve a safe and successful transition.
Days 1–30: Assessment & Planning
The first phase focuses on understanding your needs and creating a clear plan for the transition.
Services may include:
• Comprehensive initial consultation and needs assessment
• Evaluation of safety, functional status, and environmental needs
• Occupational Therapy safety recommendations (when appropriate)
• Development of a personalized 90-day transition plan
• Identification of appropriate housing, care, or aging-in-place options
• Implementation of immediate safety and support interventions
Days 31–60: Active Implementation
During this phase, we begin implementing the transition plan and coordinating services.
Services may include:
• Coordination of recommended services and providers
• Regular progress monitoring and plan adjustments
• Family and caregiver education and support
• Downsizing and move planning guidance
• Home preparation consultations when selling a property
• Real estate coordination when applicable
Days 61–90: Transition & Stabilization
The final phase focuses on ensuring stability and long-term success.
Services may include:
• Comprehensive reassessment around Day 60
• Coordination of ongoing services and support systems
• Final transition planning and completion
• Assistance with move coordination and settling into the new environment
• Handoff to long-term providers and support networks

90-Day Senior Transition Coordination Program
Investment:$900 – $1,200
This program includes comprehensive transition planning and coordination over a structured 90-day period, including assessment, service coordination, and transition support.
Concierge Transition Management
Investment:$1,800 – $2,500
For families who need hands-on transition management and full coordination support.
Services may include:
• move coordination and logistics
• downsizing and organizing guidance
• family coordination and transition meetings
• housing and real estate coordination when applicable
• post-transition follow-up and stabilization support
Additional Services (Optional)
These services can be added based on individual needs.
• Occupational Therapy Home Safety Assessment & Recommendations — $100 per session
• Transition Support Caregiver (IADL Assistance) — $35/hour
• Decluttering & Organizing Assistance — $45/hour
• Real Estate Services — negotiated separately
Real estate services are provided through Keller Williams Preferred Realty when a home sale or purchase is involved.
Private Pay Model
Operating as a private-pay service allows us to provide personalized care without insurance restrictions, prior authorizations, or service limitations.
