Post-Transition Stabilization Program

Supporting Success After Moving to Long-Term Care

Moving from your longtime home to an assisted living or memory care facility represents one of life's most significant transitions. The first weeks and months after this major change are critical for ensuring long-term adjustment, health, and well-being in your new care community. Our Post-Transition Stabilization Program offers expert support, guidance, and advocacy for up to 90 days following your move to a long-term care facility—helping you successfully adjust, thrive, and feel at home in your new environment.

Why Post-Transition Stabilization Matters for Long-Term Care Moves

Research shows that seniors moving from independent living at home to assisted living or memory care facilities face unique emotional, social, and practical challenges during this major life transition. Without proper post-transition support, the risk of adjustment difficulties, depression, or premature discharge increases. Achieving successful long-term care placement requires thoughtful ongoing assistance—not just a smooth move-in day.

The first 90 days in long-term care:

Are a period of grief, adjustment, and significant lifestyle changes

Require consistent emotional support, facility advocacy, and family communication

Directly impact long-term satisfaction, social integration, and successful aging in place within the care community

Common Challenges Seniors Face When Moving to Care Facilities:

Grief and loss over leaving their longtime home and neighborhood

Social isolation and difficulty connecting with other residents

Adjustment to facility routines and loss of independence

Family guilt and communication difficulties about the transition

Healthcare coordination gaps between home providers and facility staff

Confusion about facility services and how to access amenities
