Orientation support for family members navigating the new care community
Phase 2: Intensive Support & Engagement (Weeks 2–8)
Regular wellness check-ins and adjustment monitoring by your clinical case manager (complimentary consultation calls)
Facility advocacy to ensure your needs and preferences are being met by care staff
Social integration support to help you connect with activities, dining, and other residents
Healthcare coordination between your existing providers and facility medical staff
Additional companionship and emotional support services available at standard hourly rates
Family communication and regular updates about adjustment progress
Phase 3: Long-Term Stabilization & Community Integration (Weeks 9–12)
Ongoing facility relationship advocacy and quality of care monitoring
Social engagement assessment and activity participation encouragement

As a valued Move Management client, you have access to Post-Transition Stabilization support with no additional program fees:
Care Coordination & Consultation: Complimentary phone support and facility advocacy included with your original Move Management investment
Occupational Therapy Services: $100 per session (for facility environment assessment as needed)
IADL Companion Support: $35 per hour (for social integration and companionship during adjustment)
No additional retainer required - pay only for hands-on services you choose to utilize
